Legend holds that in the 10th year of Empress Suiko's reign, Ochibasu Mitsu discovered the spiritual efficacy of Yakushi Nyorai (Medicine Buddha) together with Nichira Shonin for the purpose of healing illness, and established the temple. During the Nanbokucho period, it served as the residence of Prince Yoshinari. In the Edo period, it obtained the temple name 'Kozouji' as the second-ranking temple in the Daiakuji assembly. In the Meiji era, it returned to being a minor branch temple of the Shingon Daiakuji sect, and is currently a direct branch temple of Kongobuji on Mount Koya.
