深大寺の西門を出て参道に下る途中にあり、鮮やかな赤色の幟が並んでいるため見つけやすい場所に位置しています。山の斜面に石造りの壁を造り、中央をくり抜いた祠の中に延命観音像が祀られており、神秘的で独特な雰囲気があります。毎月18日の午後1時からは延命息災祈願が行われています。
