It is said to have been enshrined since before 1570 CE (Genkō 1). A Shinto shrine (Shinmeisha) had been established on the grounds of Iseyama since before 1711 CE (Shōtoku 1). In 1826 CE (Bunsei 9), Shimura Bun-no-jō invited the enshrined deity of Konpira Grand Shrine (Konnōhira-gū) from Shikoku, leading to the founding of a merged shrine combining the Shinto shrine and Kotohiki Shrine. In 1980, the shrine was renamed Kotohiki Shrine, and a new main hall was completed in 2011.
