In 1891 (Meiji 24), a small main hall was constructed for Kuruyama Shrine, enshrining Sugawara no Michizane, a branch spirit of Dazaifu Tenmangu (Dazaifu Tenmangu Grand Shrine). In 1952 (Shōwa 27), the shrine was renamed Kuruyama Tenmangu. In 1969 (Shōwa 44), a new main hall (honden), worship hall (haiden), and administrative office were built on the current grounds. Additionally, Kuruyama Inari Shrine, originally established in 1923 (Taishō 12), was merged into this shrine in 1964 (Shōwa 39).
