Legend holds that the shrine originated when Sukuna-bikona-no-kami, who assisted Omononushi-no-kami in pacifying the land, established facilities to treat illnesses and injuries. In the third year of Tenki (1055 CE) during the Heian period, a spirit of Sugawara no Michizane was enshrined from Kitano Tenmangu, and the site was named 'Matsuo Tenmangu'. It was renamed 'Matsuo Tenjinsha' in the sixth year of Meiji (1873 CE). The current shrine buildings were constructed in November of the 60th year of Showa (1985 CE).
