Originally enshrining Iwade Daijin, the deity was transferred in 786 CE (Enryaku 5) by General Sakurai no Tamamaru to the current head shrine of Harima Province. Later, residents who settled at the foot of Otoko-yama built a shrine to enshrine the deity. In 1619 CE (Genna 5), the shrine buildings were constructed by Honda Minonokami Tadamasa. During the separation of Shinto and Buddhism in the Meiji era, the shrine was renamed Mizuo Shrine from Otona Shrine and Toshi Tokutai Meigami.
