It is said that in 1336 CE (Kenmu 3), when Kusunoki Masanori and others were on their way to Hyogo for battle, villagers enshrined a deity at the site where they had paid respects to a Hachiman Shrine in Wakihama. In 1873 CE (Meiji 6), it was designated as a village shrine. In 1932 CE (Shōwa 7), the current shrine buildings were constructed at the present location, and Usa Hachiman Shrine was merged into it, leading to a name change to Nan'gū Usa Hachiman Shrine. The shrine suffered severe damage in the 1995 CE (Heisei 7) Great Hanshin-Awaji Earthquake, but reconstruction was completed in 1999 CE (Heisei 11).
